Perhaps your best friend's birthday is just around the corner. You've already giver her/him a digital camera, the routine slippers, scarves, hats and sweaters etc. but this year you want to do something a bit special. Something that really says- wow, we've been friends forever and I want to show you what that means to me. Here are some ideas on how you might do that.
1. Something sentimental:
Not everyone has got such a soft side, but for those people that do, it might be a nice idea to do or make something really sweet. An idea might be to put together a collection of photos of the two of you throughout your years of friendship and then bind it nicely. You could even write a nice little note/forward and print it on the inside cover.
2. Something grand:
If it's in your budget, paying for something like a weekend trip to a fun city is a great idea. There's nothing quite like travelling with your friends, so planning and paying for a trip together, to somewhere neither of you have ever been could be super duper cool! I don't think you need to go as far as paying for the hotel and everything you do while there, just the gesture of the idea and the tickets is enough.
3. Something perfect:
If your best friend is a girl and you buy her a gorgeous dress that fits perfectly and is just her style, that says something. Things like dresses for women or nice fitting sports jackets for men are usually left to wives, mothers, and siblings to buy. The reason for this is because no friend would ever dare. You're part of the family now, so buy like a true member.
4. Something memorable:
Another idea might be to throw your friend a party. It doesn't have to be a surprise (although that might be fun), you can get them involved. But offer to host it at your house, organize the caterers, buy the booze and take care of the clean up. This might be especially nice for someone who doesn't have the space or time to manage such an event. Give them a party they'll never forget.

Is your friend a daredevil? Are you? If so you might want to get two tickets to take a hot air balloon trip. It's a great way to see a city or countryside and doesn't have to be expensive. Unlike skydiving that requires training hot air balloon flights don't have to be time investers.
Buying gifts can be tricky, especially if it's something you really want them to like. That sounds silly but so often people put no thought into the things they give and it's obvious. Giving something great requires thought and time. The ideas above though, should help get you started.
