- 1). Visit the high school guidance counselor in your school district to find out about the labor laws for teens between 14 through 17 years of age. Labor laws differ from state to state. In some areas, children as young as 12 years of age may be permitted to work after acquiring a permit.
- 2). Fill out the work permit form provided by the school counselor. Both the parent of the teen and the teen must sign and date the form. In some states, your teen may be required to already have an offer of employment before they can apply for a work permit. In this case, have the employer fill out details required from him and sign the form where indicated.
- 3). Provide proof of your child's age during the application process. This includes a copy of the birth certificate or a U.S. passport.
- 4). Schedule a medical physical for your child if necessary. In the state of Massachusetts, teens under 16 years of age are required to have a physical before they can apply for a work permit. The doctor must sign the permit, indicating that the teen is physically able to work.
- 5). Return the application to the school guidance office so they can process it before issuing the work permit.
