- 1). Visit the Oklahoma Department of Human Services website, linked in the Resources section.
- 2). Scroll down on the page and select "Apply for Foodstamps" under the "How do I?" header.
- 3). Click on the "Download applications" link.
- 4). Select the "Request for Benefits and Services (Form 08MP001E)" link. This is the form you will need to apply for food stamps.
- 5). Print out the form.
- 6). Fill out the form with accurate information. Be sure to check the box next to "Supplemental Nutrition Assistance Program (SNAP)." This is the first option on the application form.
- 7). Visit the Locate County Information page, linked in the Resources section. You will need to locate an office to drop your application off at. Alternatively, you may fax this form to your local office. To locate an office, click on the county that you reside in. A page will appear listing the phone number, address and fax number for the county office. You may click "Map it!" to bring up directions to the office.
