Form SSA-521 is the official SSA form to discontinue the receipt of benefits. Note that Form SSA-521 serves as both a form to withdraw an application as well as a form to request a cancellation for the receipt of Social Security benefits. - 2). Print and complete Form SSA-521. Be sure to complete the form truthfully and accurately.
- 3). Visit your local Social Security office to submit the form. To locate the nearest Social Security office in your area, see the Resources section. Your local office will process your request to stop receiving SSA checks.
