- 1). Fill out an Ohio WIC application. The application can be view and printed out on the Department of Job and Family Services website (see link below). You will need you and your child's social security number, along with a current address and contact number.
- 2). Make copies of required documents. Include mandatory copies of valid identification (such as valid driver's license, state ID, passport, ect), as well as proof of income (recent paycheck stubs or written income proof from your employer). If you are an expecting mother, a doctor's note stating due date and number of unborn children is required. Provide a copy - front and back - of you and your child's medical coverage (if you have it).
- 3). Review application thoroughly and make sure that you have provided all necessary copies of required documents so your application will not be delayed. Mail the application into your county's local Department of Job and Family Services. See resource box below for department addresses and contact info.
