A 22-year-old acyanotic female was admitted with pulmonary edema following a mountain hike. She reported that her exercise tolerance had always been below normal and that she had recurrent episodes of pneumonia and bronchitis during childhood and adolescence. At presentation, she had sinus tachycardia (heart rate 110 beats per minute). The ECG showed right axis deviation and notched "left atrial" p waves. A loud second heart sound and a soft systolic/diastolic heart sound were heard on auscultation.
