How to Use a Duran Splint

      See the surgeon about three days after surgery for a dressing change and medicine for edema, if needed. A custom-molded dorsal blocking splint will be applied to hold the hand and wrist in the same position. This type of splint is used to allow for Early Passive Mobilization, which inhibits restrictive adhesion formation and promotes a stronger repair of the hand.

      Wear the splint on your hand and do not remove it for four weeks after surgery. The Duran splint was designed with the purpose of removing it a few weeks after surgery for a short time each day to perform exercises to keep the joints limber and return the full range of motion of the wrist. The splint has Velcro straps that can be loosened or tightened to allow for exercises completed while wearing the splint.

    • 3). Begin the following exercises two times each day while wearing the splint without the Velcro straps:

      8 repetitions of passive bending and active straightening of the closest finger joint to the tendon repaired

      8 repetitions of passive bending and active straightening of the most distant finger joint to the repaired tendon

      The Duran Splint places the wrist in a 20 degree position of bending, with the closest and most distant joint to the repaired tendon in full extension.

    • 4). Remove the dorsal blocking splint every two hours for the following exercises after the fourth week:

      10 repetitions of active bending and straightening the wrist

      10 repetitions of active bending and straightening the fingers

      Passive exercises are continued as needed.

      Re-apply the dorsal blocking splint after exercises have been completed..

    • 5). Discontinue the dorsal blocking splint after five weeks, as prescribed by your doctor. You will begin active exercises every hour as prescribed by your doctor. Physical therapy will continue until your doctor is satisfied with your progress. No lifting or heavy use of the hand is allowed for several weeks.
