- 1). Visit the Social Security Administration website and print out form SS-5.
- 2). Fill out the form entirely making sure all information is accurate.
- 3). Gather the documents needed to prove your identity. This includes a valid driver's license or other state issued ID with a photograph, and a certified copy of your birth certificate.
- 4). Search for the Social Security Administration office closest to you and mail the identification documents and form SS-5 to the address listed.
- 5). Watch for your replacement Social Security card and identification documents in the mail. You should receive them within 10 business days.
