• 1). Visit the Kentucky division of motor carrier's website (see Resources) and print out form TC-951.
  
  
• 2). Fill out the form with the information requested with a black pen. Make sure your writing is legible.
  
  
• 3). Mail the form to:

  Department of Vehicle Regulation Division of Motor Carriers Rev. 4/10
  
  P.O. Box 2007
  
  Frankfort, Kentucky 40602-2007
  
  
• 4). Wait for your KYU number to arrive by mail in a few weeks.

• 1). Visit the Department of Vehicle Regulations in Kentucky at:

  502-564-7000
  
  
• 2). Inform the representative of your intent to obtain a KYU number. Wait for her to provide you with the TC-951 application.
  
  
• 3). Fill out the form with the information requested with a black pen. Make sure your writing is legible.
  
  
• 4). Return the form back to the representative and wait for her to process your license. Retrieve your license when told to do so.
