- 1). Obtain the 1040 tax form from either the local post office or by downloading it from the Department of Revenue website. The form can also be submitted online.
- 2). Fill out the form, including your name, address, Social Security number, your salary for the year you are filing and how much you owe in taxes, based on what tax bracket you fall into. For example, if you made $50,000, you would multiply $50,000 x 0.07 ($3,500) to compute how much you owe.
