Hysteroscopy invariably defines the diagnostic analysis of intrauterine pathology and serves as a method for surgical intervention referred to as operative Hysteroscopy. Physician's uses a thin viewing tool called a Hysteroscope for the purpose. The tip of the Hysteroscope is put into the vagina of the patient and gently moved through the cervix into the uterus. The device has a light and camera hooked to it so the doctor can see the lining endometrium on a video screen. Hysteroscope carries optical and light channels or fibers which are introduced in a sheath that provides an inflow and outflow channel for insufflations of the uterine cavity .An operative channel is also present to introduce scissors, graspers or biopsy instruments in case operation is required during Hysteroscopy. It can be compared to a transurethral resectoscope that allows the entry of an electric loop to shae off unwanted tissue, for instance to eliminate a fibroid.
Hysteroscopy has profound uses in diagnosis of abnormal bleeding or bleeding that occurs after a woman has passed menopause. It is also done at times to check if a problem in the uterus is preventing a patient from becoming pregnant, that is in finding out causes of infertility. Hysteroscopy has been widely used to remove growths in the uterus, such as fibroids or polyps.
The doctor takes a small sample of tissue to be viewed under a microscope for problems-the procedure called biopsy generally done to check for pre malignant tissues or lesions. Surgery may also be done during Hysteroscopy by laparoscopic procedure, if infertility is a problem.
Hysteroscopy has been useful in diagnosis of causes of severe cramps or abnormal wherein the doctor can pass heated tools through the device to curb bleeding. If there exists any problem in the shape or size of the uterus or if scar tissue is there or even the blockage in the uterine openings to the fallopian tubes can be surgically corrected through the device. Physicians also do Hysteroscopy to find out the reasons of repeated miscarriages, find and reposition a misplaced intrauterine device (IUD) used as permanent sterilization measure. Heated tools are generally incorporated through the device during operative Hysteroscopy to remove the problem areas in the lining of the uterus (endometrial ablation).
While Diagnostic Hysteroscopy is used to diagnose problems of the uterus, Operative Hysteroscopy is used to correct an abnormal condition that has been detected during a diagnostic Hysteroscopy. Diagnostic Hysteroscopy is used to confirm results of other tests like hysterosalpingography (HSG), an X-ray dye test used to check the uterus and fallopian tubes.
Hysteroscopy can also be used with other procedures, such as laparoscopy, or before dilation and curettage (D&C). Operative Hysteroscopy used to surgically correct the condition avoids the need for a second surgery.
Hysteroscopy has many advantages over traditional practices of obstetrics and gynecology. Diagnostics in Hysteroscopy can easily be done in an clinic setting using local anesthesia. Simple operative Hysteroscopy can also be performed in a doctor's clinic with just a paracervical block using the Lidocaine injection in the upper part of the cervix keeping the patient is in a litho to my position. It causes less pain and lesser medication after surgery and also shorter recovery time.
