How do I Extract an Original Social Security Card Application?

    History

    • The Social Security Act was enacted in 1935 but wasn't actively put into play until 1936 when approximately 30 million allocations for social security numbers were processed over an eight-month period.

    Features

    • Original social security card applications (SS-5 form) of the deceased can be ordered online or by mail through the U.S. Social Security Administration. You will need to fill out the designated application form as best as you can providing basic information regarding the applicant including the applicant's first and last name and parent's names. The fee for a copy of the original application is $27 if you know the social security number and $29 if not. A less detailed computer summary of the social security application is $18 and $16 if you do not know the social security number. The Social Security Death Index is a valuable search tool for deceased social security card applicants.

    Considerations

    • To request another living person's records, you must send your request along with the social security applicant's signed consent to your local social security office.
