When Should We Suspect Hereditary CRC Syndrome?
The revised Bethesda guidelines were published in 2004, and they have been the standard of care for selecting patients who should be prescreened with MSI and/or IHC for MMR protein expression. Abnormal results would prompt the performance of germline genetic testing. The NCCN has adopted the revised Bethesda guidelines and added endometrial carcinoma diagnosed before the age of 50 as an extra criterion for preselection with molecular testing.
Regarding cancer surveillance and prophylaxis, the NCCN has adopted basically the same approaches as the ones previously outlined. This will probably be eventually modified as more information on cancer risk, age of onset, and effectiveness of interventions from large studies and segregated by the particular responsible gene become available.
