- 1). Visit the IRS website and print either Forms 4506-T or 4506 (see Resources). Form 4506-T requests W-2 information and Form 4506 requests an actual copy of your W-2.
- 2). Fill out the printed form. Enclose a check or money order for $57.00 per W-2 requested for form 4506. Form 4506-T is free of charge.
- 3). Mail the form to the correct IRS address listed under the "Chart for individual returns" at the bottom of the form. The address varies depending on which state you live in.
