- 1). Call or visit the Birmingham office of the Social Security Administration. The agency is open Monday through Friday, 8:30 a.m. to 3:30 p.m., except on federal holidays. The address is 1200 8th Avenue North, Birmingham, Alabama, 35285-0001. It is best to make an appointment by calling 800-772-1213. Going to the office without making an appointment means you may have to wait a few hours. If you are hearing-impaired, call 800-325-0778.
- 2). If you cannot make it to the Birmingham office, you can fill out an application for Social Security Benefits and an Adult Disability Report on the Social Security Administration's website (see Resources). You must complete both of these forms to be eligible for disability.
- 3). To complete the Disability Benefit Application, provide information regarding your circumstances. You must fill in your medical or mental health information, and note whether you have one of the required medical conditions the Social Security Administration lists for disability. You must also describe how your ability to work has been affected by your medical or mental condition.
- 4). If you have any questions about applying for disability, make an appointment with the local Birmingham office to get help from a representative. You can ask questions over the phone, but you cannot make your application by phone.
