- 1). Download and complete Form I-90, Application to Replace Permanent Resident Card. This form is used if you are renewing or replacing a lost, stolen, mutilated or destroyed resident card. You can access this form at uscis.gov.
- 2). Include the initial evidence along with your application. Include a photocopy of your previous green card or government-issued identification document (such as a driver's license). Also include a check or money order for the application filing fee of $370 (biometrics fee included) made payable to the U.S. Department of Homeland Security.
- 3). Send the completed application, copy of identification and payment to:

Phoenix, AZ 85036 - 4). Wait for application approval from the USCIS. You will receive written information on where to go to fulfill the biometrics portion of the application. After that, you will receive your new Permanent Resident Card in the mail.
