Health Insurance Requirements

    Premiums

    • Health insurance premiums must be approved by the insurance commissioner in each state. A premium cannot be increased during a policy term. Increases must also be approved by the insurance commissioner.

    Policy Terms

    • Policy terms and conditions must be clearly set forth for the consumer in a policy. These provisions cannot be altered during the term of a policy absent advanced notice to the insured.

    Termination

    • Health insurance requirements in all states require providing a consumer with advanced notice before a policy can be terminated. Depending on the state, this notification period is 30 to 90 days.

    Mental Health Coverage

    • In addition to providing for physical conditions, health insurance requirements in all states mandate at least basic coverage for mental health issues.

    Warning

    • Beyond basic health insurance requirements, there are significant variations in policies. Therefore, a consumer must closely examine an individual policy before purchasing coverage.
