- 1). Make a complaint with the social worker's supervisor. If you don't know who that is, the social worker may tell you on request (though he is not obligated to do so), or you could call that particular social work division and ask to speak to a manager or supervisor. Consult the Colorado government directory to look up the number for a social work office.
- 2). File a complaint through the Colorado Department of Regulatory Agencies, which oversees licensing for and complaints against social workers and other public employees in the state. Obtain a copy online of the official complaint form.
- 3). Fill out all the information required by the form, including your contact info, the nature of your complaint, supporting details and any witnesses. Note that anonymous complaints are not accepted.
- 4). Make copies of any documents or paperwork that provide evidence of your complaint and attach to the complaint form.
- 5). Submit the complaint form and relevant documents to Division of Registrations, Complaints, 1560 Broadway, Suite 1350, Denver, Colorado 80202.
