- 1). Ask your employer about your W-2 form if you have not received it by February 1. If your employer says it was sent, double-check that your mailing address was correct. Ask for another copy.
- 2). Review your W-2 form once you have received it. Your employer's EIN is in box "B." W-2 form designs vary, but the box labeled "B" always contains the EIN.
- 3). Find a W-2 form from the employer from a previous year to get the EIN from box "B." The government uses the EIN to identify companies and the number is unlikely to change. The EIN is for companies what the Social Security number is for individuals.
- 1). Call the owner's assistant, bookkeeper or payroll clerk in a smaller company and ask for the EIN. If she does not know the number, she can find out who does.
- 2). Call the payroll department in a larger company or the person responsible for payroll and ask for the EIN.
- 3). Call accounts payable and ask for the EIN if your company does not have a payroll department. Nonprofit and wholesale organizations use the number often, so individuals who order goods for the company will have the number.
- 1). Find the pay stub from the last paycheck for the tax year that you received from the company. This pay stub contains year-to-date totals for pay received for the year and lists year-to-date totals for taxes and other withholdings.
- 2). Contact the IRS after February 16th at 1-800-829-1040 to report that you did not receive your W-2. You will be asked to provide your identification and contact information and information about the employer. Additionally, you will need your dates of employment, wages earned and taxes withheld for the tax year. The IRS will send you Form 4852.
- 3). Visit your local IRS office, or go to the IRS website and request Form 4852 if you do not want to wait for it to come in the mail. Fill out all the information to the best of your ability.
