- 1). Fill out form "10-10EZ," or application for health benefits form. Complete the form online through the VA website, or mail, fax or deliver a paper copy of the form in person to your nearest VA medical Center. Attach a copy of your discharge paperwork to the application.
- 2). Wait to receive a confirmation letter from the VA saying that you have been accepted and enrolled in the VA health-care system. This can take anywhere from two to eight weeks.
- 3). Call your local VA medical center and ask to set up an appointment for a physical exam. Prepare to give your name, the last four numbers of your Social Security number and your birthday to make an appointment. Tell the VA what kind of physical you need and why.
